Youth Centered Transition to Adult Health Care Conference

The conference will serve to explore and promote a youth-centered approach to health care transition planning.  The conference will not only serve as a call to action within the medical community regarding the necessity of person-centered transition planning, but will also be a launching off point for committees and workgroups that are focused on specific aspects of health care transitions.  We hope to draw from stakeholders for youth centered transition including clinician and administrative leaders, transition coordinators and advocates from school and community settings, and individuals and family members.  Information regarding CME, CEU and Act 48 credits will be forthcoming.
